Our Nursing Home Transition (NHT) department is currently serving 133 individuals. Since January 1st, our team has transitioned a total of 27 participants. Although transitions are down from this time last year, the desire for participants to get out of nursing facilities and back into the COMMUNITY continues to grow. |
|
|
The NHT team continues to work hard and go above and beyond to provide individuals with the best possible assistance available. With our growing numbers, we are looking to hire more remote positions to assist more people to escape the prisons of nursing facilities and integrate or re-integrate back to community living. |

Wally transitioned back to the community in February. We followed up with Wally in May to see how things were going. When staff arrived, three of his friends, whom he hadn’t seen in a few months, had stopped by to see him. They were joking and laughing, having a great time. It was nice to see Wally smiling and having a good time with his friends. Wally has been getting out into the community and doing different things like going to church, NA meetings and just spending time with his friends and family. Many don’t know this, but Wally is also a comedian. |

Wally was offered the chance to be an opening act for “Bruh Man”. At first, Wally declined the offer thinking he had been away for so long and not being able to walk he didn’t think he had the ability to do it anymore. Through the encouragement of his friends and family (or shall we say in Wally’s terms “the relentlessness of them”), he decided to give it a shot! This event was all Wally spoke about while staff was visiting. Seeing him get so excited and hearing about how involved he is in HIS community is why we love what we do! WELCOME HOME, WALLY! |

Emergency Relocation – UPDATE! As you may remember, in our last newsletter we introduced you to Jason. Jason transitioned to his own apartment on May 10th, 2022, just two days after his 38th birthday. Jason was in the hotel for roughly 64 days before securing his own apartment. Jason is enjoying being able to have a recliner of his own and sitting outside enjoying the nice weather. CONGRATULATIONS, JASON! |

Independent Living Success Story |

When Megan first contacted RTF, she was feeling overwhelmed with things happening in her life and shared she has little support from her family. RTFCIL staff assisted Megan to prioritize her needs and identify the barriers that were preventing her from achieving success with her goal of independent living. Megan began chipping away at items, one at a time. Megan was clear that she wanted to remain in a rural area but was feeling a bit defeated after much time looking for an apartment or home that would meet her personal and financial needs. Megan was beginning to feel her only option would be an apartment in an urban area. Megan was excited to share that a rental opened in Muncy, approximately one mile from where she was currently living. Megan stopped at the home to get the contact information and the property owner was there. Megan spoke with him, explained the voucher program, and asked if he would consider having the Lycoming County Housing Authority approve the rental for the voucher program. The owner agreed! Megan now had a home that would meet her needs and budget! Megan was quick to thank the RTF team for her success. Staff reminded her that our role was minimal by comparison; the team offered guidance and support, but it was Megan who put the work into making her person-centered plan a reality. Roads to Freedom is a proud provider of Representative Payee Services, also known as Rep Payee Services. We help those who need assistance managing their finances by paying bills such as rent, groceries, electricity, gas, phone, and cable as well as giving a stipend to the consumer. RTFCIL employees develop a budget, assist with understanding social security administration letters and payments, as well as report changes and paystubs to the Social Security Administration. Most of our consumers are receiving disability benefits; however, we also offer Financial Power of Attorney services for those who work and are not receiving disability benefits.
