The Larimer Advance Care Planning Team has had a fruitful Summer season filled with community events, one-on-one appointments, and several employee campaigns. We are sad to see the sun going down earlier, but so happy to have had an eventful summer! |

With the help of two amazing interns, the Larimer ACP team, along with UCHealth and the Sharing the Care Campaign, launched the first Patient Passport event in June. The Patient Passport is a free, convenient, portable, user-friendly tool which is similar to a travel passport, but this one can be used by people (including those with chronic illnesses) as they ‘travel’ through the healthcare system. It helps communicate essential health information, special needs/preferences, communication approaches, and informed healthcare wishes to healthcare providers. The interns also created a toolkit to help guide future events. We could not have had a successful launch without the hard work and time that the two CSU interns, Nisha and Jesse dedicated to our program! Kudos to them for setting up this initiative for success.
