Home Stretch WA in stories

One of the great privileges of working on the Community of Practice is that we get to hear some of the amazing stories of work being done across the state with young people receiving support from Home Stretch WA.

 

Yorganop reflect on 12 months of Home Stretch WA referrals...

 

In August, Coordinator Gareth Morkel from the Yorganop Home Stretch WA team got in contact to celebrate and acknowledge his team's efforts. Yorganop have been instrumental in helping the design, development and roll out of the Home Stretch WA model and have provided the trial team with incredible insight in how the model can support young Aboriginal people leaving care.

 

Here's what Gareth had to say about the experience of leading his team through the first 12 months of service...

"I’m extremely proud that we have such a strong team who have all moved mountains with these incredible Young People. You have achieved so much in such a short period of time and taken this program on with everything you have mentally and physically and you all place the Young People at the centre of everything you do.

I don’t think we can take credit for transforming those Young People’s lives because they’ve been active participants as agents of change in their own lives… but you’ve all been the catalysts by sitting with them and roughing our the tough times and encouraging them to take on more and to become more than they had previously thought possible."

The wins we experience as Home Stretch WA providers, no matter the size, are what keep us energised and inspired, so if you have any stories or experiences to share with us post them in the forum!

What has the YAG been up to?

The YAG (Youth Advisory Group) met today over what looks like a scrumptious lunch, to talk all about Home Stretch WA, ensuring the voices of young people are heard loud and clear, towards continuously improving the service.

That's all folks...

The Home Stretch WA Trial and the Community of Practice Team would like to acknowledge and farewell Ben Dancey & Hayley McKenzie as the Home Stretch WA Trial comes to a natural end this year. Both Ben & Hayley are super talented Youth Workers and Leaders so it's no surprise that both have been snapped up by Anglicare WA Youth Services and will continue to support young people in the sector.

 

Ben will be utilising his person-centred leadership skills and is moving over as Coordinator of Anglicare WA Youth Psychosocial Support Program. Hayley will be moving to Foyer Oxford as a Case Manager, continuing to provide intentional support to young people and practice leadership. Ben and Hayley are both taking some well-deserved annual leave as they transition into their new roles. Ben's last day before leave is Friday 8th September and Hayley will finish up on Thursday 21st September.

 

Ben & Hayley have both achieved numerous individual outcomes with young people over the past 4 years and have been instrumental in developing the Home Stretch WA model, supporting the statewide roll out of the program through training, practice hours and mentoring support and building a collaborative vision for young people leaving care.

 

Thank you both for your support to young people, the out-of-home care sector and the Home Stretch WA teams! You have left a legacy and should both be so proud. We will miss you…but glad to have you still in the sector supporting young people.

What will happen to the Practice Hours?

The Practice Hours will continue to be supported by the CoP, moving more towards a guest speaker format, with practice specialists. Provider teams will also be invited to facilitate group discussions about supporting young people, focusing on different practice areas.

Sortli (short for ‘sort out your life’) is the ultimate guide to becoming an adult. Erin, from the Create Foundation, who manage the app, will provide all the information you need to know about how to support young people to use the app.
