D-PATH is an inclusive, educational, health promoting project targeting three groups: people with diabetes and metabolic diseases, related families and educators. It aims to look into the needs of these groups with a purpose to help the first two become socially included with support from the latter. To this end the partnership planned the overall research and needs assessment to enable the development of a board game, adaptation of a number of training sessions and support activities. Moreover, awareness raising on nutrition and metabolic diseases and two project meetings in Poland and Romania will also be included . Last but not least promotion and dissemination of the project results internationally, at an EU level and in the partners’ countries will enhance the D-PATH impact and follow up. The outcomes planned for delivery by the end of the project in September 2023 include a research report in the field of dealing with diabetes and metabolic diseases, a board game for diabetics to understand their own needs, problems and solutions, opportunities and risks, raising awareness Lego +robotics tools to show the chemical reactions in the human body as related to nutrition. In addition a blog will enable sharing advice, solutions and back up for the people with diabetes or pre-diabetes, their families and educators with a purpose to help them cope with everyday life difficulties. A variety of identity, promo materials, e-publications and videos will make the outcomes visible to the diabetes communities and beyond. The First Project Meeting |

The first project meeting took place online to say hello, go through the project tasks and set the ground rules for the next few months in view of preparation for the participation in the face to face project meeting to take place in the framework of the D-CAMP, August 2022 at Przemysl, Poland. |

The photo shows the representative of the four partners involved in the project: FUNDACJA WYMIANY KULTUROWEJ TO TU TO TAM - Poland the coordinating organization represented by Magdalena Pardel–Trefler with partners from Greece represented by Antonio Russo, Romania represented by Viorica Alexandru and Sweden represented by Neil Bell. The partnership discussed about joining efforts in the process of target groups needs assessment so as the Swedish and Greek partners would be able to prepare the D-Path game prototype and awareness raising workshops to be put to practice with the participants in the D-Camp. Moreover, the dissemination partner put forward the design and delivery plan for the D-Path identity and promotion materials, cooperation expectations from the partners as well. |

The key words describing their outstanding activity in the 6 years preceding this project are social inclusion, migrants, disabled, minority groups. They cooperate consistently with the Association of Therapy by the Art "Katarsis" established by the parents of kids and adults with mental disability as well as with the Marek Kamiński Foundation - the Life Plan Academy creator . Magdalena Pardel-Trefler , President of toTU toTAM is a trainer of Life Plan Academy and trains the trainers of people at risk from the Polish Diabetes Association (PSD) department in Rzeszów. Magdalena is a member of Women's Council in Przemyśl and thanks to it she is in close contact with different target groups and widely aware of their needs . The coordinating organization was awarded a grant by the Norway Grants, Regional Program in 2022 for the organization of the diabetes camp for people from the Subcarpathian region in the summer of 2022 which enhance the effect and outcomes of D-Path once they would cross-fertilize . Last but not least, Magdalena , the president of the coordinating organization has had diabetes for 8 years now which makes her more aware of the situation. The coordinating organization is properly qualified to lead the project, make it sustainable and growing . bMECTE …the Winner of the 2017 Eurodesk Network prize…now developing a strategic board game under D-PATH |

The objective of bMECTE bmecte.eu is to create a dialogue, spread information, encourage discussion and provide information around the challenges we face as a race like migration, pollution and the likes. They are involved in art and educational projects and have experience in developing board and card games with an impact . They have developed projects as members of the Anna Lindh Foundation . They play the main role in D-Path because they support the main deliverable : the D-Path strategy board game . They have great experience in board games on different world issues such as migration and social inclusion, a mix that they are using to create the D-Path strategy board game mainly for diabetics, their families, educators of different ages . There are three stages of their task : preparation of the preliminary version , testing it with target groups in camp conditions and presentation of its final format during an event at the end of the project in Romania after undergoing changes according to the findings during the camp . |

O3 Out of Ordinary are developing workshop modules based on the use of Lego and Robotics to explain to youth chemical reactions that happen in the human body of those people who are suffering from diabetes and metabolic disorders. The activity involves experts in nutrition, robotics, STEAM, and ICT. Since 2019, they have been implementing innovative educational programs in which learning is combined with fun. Their main focus is on creative play that exalts imagination and creativity and with years of experience in education, they pass on knowledge and new methods to the educational community, changing the lives of future generations. Additionally, O3 hold experience and expertise in digital skills and projects (digital literacy, coding, educational robotics, etc), 21st-century skills (soft skills, life skills, social skills, etc), STEAM skills (Science, Engineering, Technology, Arts, Mathematics), workshops and training (of teachers, staff, parents and students alike), as well as research and dissemination. |

Initiativa is an association run by seniors using the seniors' professional expertise and life experience to the benefit of the community. During the 13 years of existence there has been a steady support for active aging through ICT learning, socializing on FB and other media as well as participating in learning events. Health and eco- campaigns to the benefit of disadvantaged individuals and the community have been run under different Erasmus projects. Colleges, NGOs (youth, seniors and special needs), the county heritage centre and public library, mayors’ office are usually involved to enhance project impact and ensure follow –up. Therefore, Initiativa has access to a considerable pool of volunteers, educators and students (6-18) created while doing non-formal learning activities such as public speaking, drawing and photo voice competitions, flash mobs, internet incursions in the history of money, medicine and medical profession, fashion and public services, etc. Initiativa developed promotional materials and published e-books, educational guides and handbooks for local and European use within different projects. Under D-PATH Initiativa handle the dissemination WP including the dissemination strategy, promotional tools and activities in cooperation with the other D-PATH partners. International and European networking under D-PATH |

One purpose of the D-PATH dissemination strategy is spotting international and European networks benefiting people with diabetes, families affected by diabetes and educators. The aim is accessing up to date information for the partnership and tapping D-PATH to the international and European diabetes facilities. The International Diabetes Federation (IDF), an umbrella for 230 national diabetes associations in 170 countries work to meet the needs of ‘people with diabetes and those at risk; families affected by diabetes; governments; health professionals; pharmaceutical and other industries; education and research institutions; employers; and the wider community.’ Diabetes 1 and 2, prevention and complications are in the focus of the IDF activities such as advocacy, education, health care, research and events at a global, regional and national level .
