CoP End of Year Message It has been a busy year for all of us across the Community of Practice membership, and we are deeply grateful for the warmth and kindness shown by CoP members across the state who have welcomed us into their services and teams and worked closely with us to establish Home Stretch WA across the state. After a very successful year, the CoP backbone team will be on leave from the 25th December- 2nd of January, in line with Anglicare WA shutting down its services during that week. A number of staff will be taking some additional leave after Christmas but most of the team will be back on deck in early January. We'd like to take this opportunity to congratulate and thank everyone for their efforts in onboarding young people into Home Stretch WA in 2023 and wish you a safe and happy Christmas. We look forward to working with you in 2024. |

The Community of Practice is running a competition for young people aged 12-25 who have experience with Out-of-Home Care in WA. Young people can design a poster, flyer, a diagram, a piece of art or even
a Tik-Tok video that describes what Home Stretch WA offers. |

In Early December we farewelled Kristina Radcliffe from the Community of Practice team. Kristina did an amazing job as the Aboriginal Practice Lead employed by Yorganop, but working with the CoP, helping our practice community grow stronger in its understanding of how best to support young Aboriginal people, and to ensure that culture and community remained at the heart of every step and stage of the Home Stretch WA service model for young Aboriginal people. Kristina has worked hard since March 2023 to bring together Aboriginal staff in the Home Stretch WA team from all over the state and assisted in establishment workshops for new services in the Pilbara and Midwest. Kristina's practice knowledge was an important part of moving the NW Practice Hour into a Cultural Practice Yarning session for all Aboriginal staff, a forum that has allowed individuals and teams from our remote and regional ACCO members to share their challenges, experiences and collective wisdom with each other and the CoP team. This group will continue and be the foundation for the ongoing development of the Cultural Practice Framework. While we are sad to see her move on, Kristina is moving (was poached) to Life Without Barriers to take on a senior leadership role where she will oversee all of LWB services across the state from an Aboriginal perspective and will lead the development of the governance framework to support their transition to being delivered by ACCO in the future. Kristina has been a great inspiration and source of support to many of us and we wish her the best in her new job. |

We are very happy to welcome Julia Gath into the Home Stretch WA Community of Practice team, taking on the important Social Impact Project Officer role within the AWA/Yorganop backbone team and based in Osborne Park. We are very fortunate to have Julia join the CoP, she brings a multitude of skills and experiences in community services leadership, co-design, community development and community capacity building. Julia has worked for nonprofits in England, India and Nepal, and recently at AnglicareWA as the coordinator of Job Hive, a Human Centred Design project that was focused on co-designing innovative approaches to providing young people experiencing homelessness support around employment. Julia is looking forward to applying her gifts and skills to the role as Social Impact Project Officer with warmth and an open heart and encourages meaningful conversations in order to learn from experiences and keep amplifying the voices of young people. |

Evaluation Quantum Consulting Australia have been contracted by the Department of Communities to conduct an evaluation of the Home Stretch WA program. Young people, carers, providers and the Community of Practice backbone team & Youth Advisory Group (YAG) will all be participating in the evaluation. See here for all the details of the Evaluation of Home Stretch WA by Qantum Consulting. It is really important that the voices of young people are heard through the evaluation survey.
