2022 sure was busy! This past year, we have helped more than 60 individuals move back into the community through our Nursing Home Transition program. You can see some of their smiling faces HERE. The number of people wanting to leave congregate settings continues to grow. As a result, we have had to expand our nursing home transition staff. We have hired three new staff members, which offers us the best availability to cover 38 counties throughout Pennsylvania. If you are interested in learning more about the NHT program, you can contact us or check out our website. Through our Assistive Technology department, we were able to provide 87 individuals with 71 pieces of reused equipment, which saved Pennsylvanians almost $50,000. Our reuse inventory is based on the donations we receive. If you have gently used equipment, such as wheelchairs, walkers, shower chairs, etc., we will gladly accept the items and get them to the people in need of them the most. Fundraising has looked different for the past couple of years, we have had to get creative to keep everyone safe and healthy. During 2022, we held five different fundraisers which raised just under $4,000. The funds we raise go towards our Ramping Up Life program which provides individuals with accessibility to their homes by means of having a ramp installed or other needed accessibility equipment such as a stair glide. You can donate to the Ramping Up Life Program HERE. Check out the video to see a little bit of what we've been up to in 2022! |

The report states: "Promising pilot programs, such as the Roads to Freedom, Atlantis Center, and the Hospital-to-Home program, offer glimpses of potential strategies that {...} could divert more people from congregate care facilities and move those already living in these facilities out into the community." You can read the whole report HERE. |
